    What "Methylated" Actually Means

    Vitamin B12 comes in a few different forms, most commonly cyanocobalamin (the standard, most widely used form) and methylcobalamin. According to the National Institutes of Health Office of Dietary Supplements, methylcobalamin is one of the two metabolically active forms of vitamin B12, while other forms like cyanocobalamin and hydroxocobalamin only become biologically active after the body converts them. In other words, methylated B12 skips a conversion step that other forms require.

    Why It's Often Recommended for MTHFR

    This conversion step is exactly why methylated B12 comes up so often in conversations about the MTHFR gene variant, which affects how efficiently the body handles methylation — a chemical process B12 is directly involved in. For people who process methylation less efficiently, delivering B12 in an already-active form is the theoretical appeal: there's simply less biochemical work required to put it to use.

    Worth knowing: This is a genuinely evidence-based reason to choose methylated B12 — but it's also where honest sourcing matters. A peer-reviewed comparison published in a 2024 review found that animal studies showed methylcobalamin was retained in the liver at meaningfully higher levels than cyanocobalamin, with less lost through urinary excretion. At the same time, a separate peer-reviewed analysis in Molecular Nutrition & Food Research concluded more cautiously that coenzyme forms of B12 are not clearly superior to cyanocobalamin or hydroxocobalamin specifically for preventing or treating a diagnosed B12 deficiency. The NIH's own consumer guidance is similarly measured, stating plainly that research has not shown that any form of supplemental B12 is better than the others for the general population.

    So What's the Honest Takeaway?

    Put simply: methylated B12 has a real, biologically sound rationale — it's already active, it doesn't require conversion, and some evidence suggests better retention. But it isn't a settled fact that it outperforms other forms for everyone, and the strongest, most consistent finding across the research is the same one that applies to B12 generally — it makes the most difference for people who are actually low in B12 to begin with. If your levels are already normal, the form matters far less than whether you need it at all.
